Health Reimbursement Arrangement (HRA)
If you are an Allied (Anthem) medical PPO plan member, you will not have an HRA offered to you through our Allied (Anthem) $500 deductible PPO plan, or our Allied (Anthem) $1,500 deductible PPO plan. These plans are not compatible with an HRA, so please be sure to review the benefits to ensure you enroll in the correct plan. These plans have been designed with low copays to keep your healthcare affordable.
For currently enrolled Kaiser HMO members, a Health Reimbursement Arrangement (HRA), is still provided to you. There are no new Kaiser HMO plan enrollments as of January 1, 2026. The Kaiser HMO with HRA plan uses funds provided by Five Keys to reimburse you up to your deductible amount for healthcare expenses for covered services.
Important Notes for Kaiser members:
- The HRA will pay for Kaiser covered services up to the deductible amount provided by in-network doctors. The HRA, nor Kaiser HMO, does not allow coverage for out-of-network doctors.
- Kaiser will process your HRA claims for covered services.
- Kaiser members will not have, or need, a HRA debit card as providers are paid directly.

Five Keys Contributions to Your HRA
Employee:
Up to your medical plan in-network deductible
- Kaiser: $2,000
Employee + Family:
Up to your medical plan in-network deductible
- Kaiser: $4,000
Portability
Account funds can only be used while you are covered on the HRA plan. If you leave the company and don’t elect COBRA continuation coverage, you will no longer have access to any accumulated funds.
If you leave the company and elect COBRA continuation coverage, you will continue to have access to the HRA. Plan costs will be provided to you on the COBRA election notice from Paycom within 45 days following loss of coverage.
Using the HRA
The company contributes to your account
$2,000 for individual employees; $4,000 employee + family.
Only funds up to deductible, any amounts over deductible are member responsibility.
Your expenses are paid by your HRA
Your HRA pays your eligible deductible and coinsurance amounts.
You make all applicable copayments at the doctor’s office
These payments apply toward your deductible.
You pay your deductible
After you use all of your HRA funds, you then pay the rest of the deductible amount out of your own pocket.
After that, you pay only coinsurance
Once you have met your deductible, you share in the cost of the expenses. This is called “coinsurance.”
Please note: Funds available for reimbursement are limited to the balance in your HRA.
