Wellness Program
The Wellness Program is for all active, regular employees, full-time or part-time. On-call and temporary employees are not eligible for this program.
Documentation and receipts should be emailed to our payroll department at payroll@fivekeys.org.
All receipts must be submitted by December 31. Receipts submitted after the calendar year are NOT reimbursable.
All subsidies and reimbursements are paid as TAXABLE WAGES.
Gym / Studio Membership Subsidy
What is it?
Five Keys will provide financial assistance with the cost of your gym/studio membership.
The school will reimburse employees $100 against the annual cost of gym/studio membership.
You may join any gym or workout studio you wish and with a copy of the contract you may request a $100 annual reimbursement.
When is it?
If you already belong to or wish to join a gym you can receive the benefit when you provide documentation. Each year you can apply for another $100 reimbursement.
How do I get reimbursed?
Submit a receipt to payroll@fivekeys.org from the provider and receive up to $100 reimbursement.
Annual Massage Program
What is it?
After 6 months of employment, Five Keys will reimburse up to $50 for a therapeutic massage.
When is it?
Once a year.

Smoking Cessation Subsidy
What is it?
Participation in a group class or program, visits to a certified hypno-therapist or other licensed practitioner designed to assist smokers in quitting will be supported on an individual, as-required basis.
When is it?
If the class or program cannot be scheduled during non-work hours, employees will be allowed to take the time away from work as sick time, which will be deducted from their balance. Upon approval by Human Resources, the paid time will be entered as sick or PTO, depending on what the employee has available.
Where does it take place?
This is an off-site activity, scheduled accordingly.
How much is it?
Most of the non-profit and county sponsored programs are free. If you cannot find a free smoking cessation program, Five Keys will contribute up to $100 for the program.

Weight Watchers Program Subsidy
What is it?
Five Keys will provide financial assistance with the cost of your Weight Watchers or similar weight loss membership / program. The school will reimburse employees $100 against the annual cost of weight loss membership / program.
When is it?
Please schedule meetings before or after work.
How do I get reimbursed?
Submit a receipt to payroll@fivekeys.org from the provider and receive up to $100 reimbursement.
Athletic Event Participation Subsidy
Five Keys will contribute up to $50 to support organized events such as walk-a-thons, foot races, triathlons, etc., for which the employee is raising charitable donations. This contribution can be used for event registration or as a donation for events requiring a minimum amount of contributions. This benefit can only be used for non-political, non-religious, and non-commercial events. Limited to one event per employee per calendar year.
